ABSTRACT

Objective:To explore any effect of upper limb swing training guided by rhythmic auditory stimulation (RAS) on the walking ability of stroke survivors.Methods:Eighty stroke survivors were randomly divided into an observation group and a control group. Both groups received conventional rehabilitation treatment, including neuromuscular facilitation, muscle strength training, balance training and gait training, but the observation group was additionally provided with RAS-guided upper limb swing training for 20min once a day, 5d per week for 6 weeks. Before and after the intervention, balance and lower limb function were quantified in both groups using Holden′s walking function classification, the Fugl-Meyer lower extremity motor function scale (FMA-LE), the Berg Balance Scale (BBS) and the 10m walk test (10MWT). Limits of stability were also quantified.Results:After the treatment, the average Holden, FMA-LE and BBS scores, as well as the average 10MWT time were significantly better in the observation group than in the control group. The average stability limits and their maintenance were also superior.Conclusion:RAS-guided upper limb swing training can improve the gait, walking ability, walking stability, walking speed and balance of stroke survivors.

ABSTRACT

Objective:To observe the effect of combining respiratory muscle training with occupational therapy in rehabilitating the upper limb function of stroke survivors.Methods:Fifty stroke survivors with upper limb dysfunction were randomly divided into an observation group and a control group, each of 25. Both groups were given routine rehabilitation treatment including proper positioning of the affected limb, physical therapy and motor function training. The observation group also received progressive resistance training of the inspiratory muscles and respiration control training combined with occupational therapy twice daily for 4 weeks. The trunk control test (TCT), Berg balance scale (BBS), Fugl-Meyer Upper Extremity Assessment (FMA-UE), Action Research Arm Test (ARAT), Modified Ashworth Scale (MAS) for the Hemiplegic Upper Limb and the Modified Barthel Index (MBI) were used to assess the core stability, balance, upper limb functioning, upper limb muscle tension and ability in the activities of daily living of all of the subjects.Results:Before the treatment there were no significant differences in any of the indexes between the two groups. Afterward the average TCT, BBS, FMA-UE, ARAT, MAS and MBI scores of both groups had improved significantly, but the improvements were all significantly greater in the observation group.Conclusions:Combining respiratory muscle training with occupational therapy can further improve the function of the upper limbs and daily living ability beyond what is observed with traditional rehabilitation therapy after a stroke.

ABSTRACT

Objective:To observe the effect of respiratory muscle training on autonomic function and the emotional state of persons with post-stroke depression (PSD).Methods:Fifty-two PSD patients were randomly divided into an observation group and a control group, each of 26. Both groups were given routine drug therapy, physical agents therapy and comprehensive training for hemiplegia, but the observation group additionally received progressive inspiratory muscle resistance training and respiration control training twice a day. Before and after four weeks of treatment, the autonomic functioning, emotional state and the ability in the activities of daily living (ADL) in both groups was evaluated using dynamic electrocardiography, the Hamilton Depression Scale (HAMD) and the modified Barthel index (MBI).Results:After the treatment, the average heart rate variability index in the observation group, the standard deviation of 24-hour R-R intervals(SDNN), the root mean square of successive differences of R-R intervals (RMSSD)and percentage of NN interval deviations greater than 50ms and the high-frequency power (HF) were all significantly higher than before the treatment and significantly higher than the control group′s averages. The average HAMD and MBI scores of the observation group were also significantly better.Conclusion:Breathing training can usefully supplement traditional rehabilitation interventions to alleviate post-stroke depression. It will also improve their autonomic nerve function and ADL ability. Such combined treatment is worthy of promotion and wider clinical application.

ABSTRACT

Objective To observe the effect of phrenic nerve electrical stimulation combined with respiratory training on the pulmonary function,trunk stability and balance of stroke survivors.Methods Thirty-six stroke survivors were randomly divided into a control group and a training group,each of 28.Both groups were given routine rehabilitation treatment (including physical agent therapy and combined training of the limbs),while the training group was additionally provided with phrenic nerve electrical stimulation combined with respiratory training once a day for six weeks.Before and after the treatment,the forced vital capacity (FVC),forced expiratory volume in one second (FEV1) and peak expiratory flow (PEF) of both groups were recorded.The Sheikh trunk control scale (TCT) was used to quantify their trunk stability,and balance was quantified using the Berg balance function scale (BBS).Results After six weeks of treatment the average FVC,FEV1 and PEF values of the training group were significantly higher than before the treatment,and significantly better than those of the control group after the treatment.Moreover,significant improvement was observed in the average TCT and BBS scores of both groups after the intervention,with the training group's average significantly higher than that of the contrcl group.Conclusion Phrenic nerve electrical stimulation combined with respiratory training can further improve the pulmonary function and balance function of stroke survivors as a supplement to routine rehabilitation treatment.Therefore,it is worth popularizing in clinical practice.

ABSTRACT

Objective In order to analyze of poisoning causes,a new method was established utilizing hydrophilic liquid chromatography-tandem triple quadrupole mass spectrometer (HILIC-MS/MS) coupled with dispersive solid phase extraction for rapid qualitative and quantitative analysis of tetrodotoxin in nassarius and shellfish.Methods Sample (1.0 g) was extracted with 0.1% acetic acid in boiling water bath,purified by dispersive solid phase extraction with 50 mg hydrophilic-lipophilic balance (HLB),5 mg graphitized carbon black (GCB) and protein precipitation with acetonitrile,and then filtered through a polytetrafluoroethylene (PTFE) membrane.The analytes were separated on a HILIC column,and detected in selected reaction monitoring (SRM) mode via positive electrospray ionization.The matrix matching and external standard method was used for quantification.Results Tetrodotoxin showed good linearity in the concentration range between 2.0 and 40.0 ng/ml,the correlation coefficient was higher than 0.999.The detection limit of tetrodotoxin in seafood was 10.0 pg/kg.The rates of recovery varied between 74.2% and 87.9% with relative standard deviations from 2.3% to 9.1% at spiked concentrations of 25,100 and 200 pg/kg.The proposed method was applied in the detection of tetrodotoxin in shellfish and nassarius from coastal cities of Zhejiang Province.Conclusion The method was accurate,fast,easy to operate,which could meet the requirements of public health emergency testing or routine testing.

ABSTRACT

Objective To investigate the expression of brain-derived neurotrophic factor (BDNF) and nerve growth factor (NGF) after cerebral infarction and treatment with curcumin combined with electroacupuncture.Methods Middle cerebral artery occlusion was successfully introduced to simulate cerebral infarction in 60 healthy,male Wistar rats.They were then randomly divided into a control group,an electroacupuncture group,a curcumin group and a curcumin combined with electroacupuncture group (the observation group),each of 15.After the modeling,the four groups were given the planned interventions for 2 weeks,with the control group receiving no intervention.All of the rats were sacrificed after neurobehavioral scoring and immunological staining of brain tissue from the infarction area was used to detect the expression of BDNF and NGF.Results The average Bederson score in the observation group was significantly lower than those of the other groups.Cells expressing BDNF and NGF were significantly more numerous in the observation group than in the other three groups.Conclusion Curcumin combined with electroacupuncture can significantly improve the expression of BDNF and NGF in the infarcted area,and this may play a neuroprotective role.

ABSTRACT

Objective To observe the effect of respiratory training on post-stroke dysphagia.Methods Forty-two stroke survivors with dysphagia were randomly divided into a control group and an observation group,each of 21.Both groups were given routine swallowing training,while the observation group was additionally provided with respiratory training once a day for 4 weeks.Before and after the treatment,the Kubota drinking water test (KDWT) was administered and a videofluoroscopic swallowing study (VFSS) was performed to assess swallowing function.Forced vital capacity (FVC),forced expiratory volume in one second (FEV1) and peak expiratory flow (PEF) were also measured to assess the subjects' pulmonary function.Results After the treatment,the average KDWT and VFSS scores were significantly better in the observation group than in the control group.The effectiveness rate of the observation group (85.71%) was significantly higher than that of the control group (61.90%).After the intervention,the average FVC [(3.57-±0.48)L],FEV1[(2.83±0.49) L/s] and PEF [(5.36±1.04) L/s] of the observation group were significantly better than those before the treatment and those of the control group after the treatment.Conclusion Swallowing training supported by respiratory training is superior to swallowing training alone for improving the swallowing (and pulmonary) function of post-stroke dysphagia patients.Such combined treatment is worthy of application in clinical practice.

ABSTRACT

The effects of quercetin-3-O-β-D-glucuronide (Q3GA)on the triglyceride metabolism and oxidative stress in steatotic HepG2 cells and the underlying mechanism were investigated in this study.Significant fat accu-mulation was documented by Oil Red O staining;intracellular triglyceride levels were detected by triglyceride(TG)enzymatic assay.DCFH-DA staining assay was performed to observe reactive oxygen species (ROS)pro-duction of HepG2 cells.The level of malondialdehyde(MDA)and superoxide dismutase(SOD)were assayed by thibabituric acid method and xanthine oxidase method.Changes in the mRNA expression of peroxisome prolifera-tor-activated receptorα(PPARα);carnitine palmitoyltransferase 1 A(CPT1 A);medium chain acyl-CoA dehydro-genase (MCAD);cytochrome P450 4A11(CYP4A11)and acyl-CoA oxidase(ACO);which are related with fatty acid oxidation were assessed by RT-PCR.Our results showed that Q3GA obviously reduced fat deposition and TG content.At the same time;Q3 GA decreased MDA content and significantly increased the SOD activity with reduced ROS production.Moreover;the PPARα;CPT1A;MCAD expression-related fatty acid βoxidation was upregulated with the treament of Q3GA;while without any change of the expression of CYP4A11;ACO.In conclu-sion;Q3GA prevents FFA-induced HepG2 cell steatosis;and enhances mitochondrial fatty acidβoxidation;which may partly be related to its anti-oxidation ability.

ABSTRACT

Objective To investigate the apoptosis mechanism induced by BH3 mimetic S1 in human leukemia cell line K562.Methods Cell viability was detected by XTT to S1 in leukemia cell line K562.K562 cells was incubated with S1 for different time,the apoptosis rate of K562 cells was determined by flow cytometry analysis.Caspase-3,-8,and-9 activities were measured by absorption spectra.Co-immunoprecipitation was used to analyze the releasing of bax,bak from bcl-2 and mcl-1.Results Compared with control group,a dose-dependent increase in apoptosis coincided with a dose-dependent decrease in cell viability following S1 treatment suggested that S1 inhibits cell proliferation through the induction of apoptosis.The IC50 value at 24 h for S1 was 13.5 μ mol/L.Exposure of K562 cells to S1 for 12 h resulted in a time-dependent increase in FITC-Annexin-positive/PI-negative early apoptotic cells.The strong increase of FITC Annexin/PI doublepositive cells after a 24 h treatment indicated a shift to late apoptosis.S1 activated Caspase-3 and-9,but not Caspase-8 indicated that S1 induced K562 cells apoptosis via the intrinsic pathway.K562 cells treated with 5 μmol/L of S1 showed a disruption in bcl-2/bax,mcl-1/bak complexes after 8 h S1 treatment.Conclusion The main mechanism that S1 induces K562 cells apoptosis might be through the inhibition of bcl-2/bax,mcl-1/bak complexes dissociation.

ABSTRACT

Objective To evaluate the therapeutic effect,as well as the safety,of on-demand sildenafil on men with idiopathic ED.Methods One hundred and four men who met the diagnosis criteria of idiopathic ED were recruited into 2 groups.Among them,46 men who refused or were reluctant to take pharmacotherapy were recruited into the clinical control group with instruction of sexual knowledge and skills.Fifty-eight men who were involved in the on-demand group,were administrated sildenafil 50 mg just 1h before the coitus at thebeginning,then,if needed adjusted the dosage according to the effect and tolerance of the first four administration.The questionnaire of IIEF-5 was completed at the baseline and the end point,and the clinic total effective rate,the fully recovery rate of ED and the adverse events were also evaluated at the end of administration.Both groups were treated for 3 months.The therapeutic effect was reevaluated 1 month and 3 months later,respectively.Results After 3 months treatment,scores of IIEF-5 at the baseline and the end point for control group and on-demand group were as follow:5.2 and 12.6,4.2 and 19.8 ;and the fully recovery rate of ED for each group were as follow:12 (26.1%),56 (96.6%) ; the total effective rate for each group were 27.7%,97.2% ; 1 month and 3 months after the end point,the fully recovery rate of ED and the total effective rate were evaluated separately,for 1 month:8 (17.4%),56(96.6%) and 19.4%,97.1%; for 3 months:7 (15.2%),55 (94.8%) and 16.4%,96.4%.The side effects in on-demand group (16,27.6%) was little higher than the control group (5,10.9%) (P >0.05).Most of the side effects were not only subtle and transient but also would vanish with the time.Accordingly,no one discontinued because of side effects.Conclusions Treatment of idiopathic ED with sildenafil was effective and safe.In terms of the fully recovery rate,total effective rate and effect maintenance,the on-demand group at the end point,even 1 month and 3 months after the end point,showed a great advantage over the control group.

ABSTRACT

ObjectiveTo assess the cause of tadalafil failure and the feasibility of successfully rechallenging nonresponding patients.Methods A total of 80 consecutive erectile dysfunction ( ED ) patients who claimed poor response to tadalafil were enrolled into the study.A self-administered tadalafil-use questionnaire composed of eight questions was applied to assess how they had used tadalafil.Subjects were given thorough instruction based on individual answers and four doses of tadalafil 20mg.After a 2-week follow-up,end point efficacy of rechallenge was evaluated using the sexual encounter profile (SEP),which was recommended by international advisory panel in 2004.ResultsA total of 45 subjects had one or more areas of major suboptimal use of tadalafil:21.2% did not know that sexual stimulation was necessary for tadalafil to work,87.5% attempted to use tadalafil less than four times,57.5% took a maximal dose less than 20 mg,and 84% felt nervous or anxious.Of the 65 patients undergoing tadalafil rechallenge,30 patients answered “yes” to SEP2 and SEP3.The response rate to rechallenge was 46.2% ( 30/65 ).ConclusionsInappropriate use of tadalafil was major cause of tadalafil non-pesponse.The efficacy of tadalafil could be improved to a better extent by education of patients.

ABSTRACT

Objective To estabalish a new conception, Erectile Dysfunction-no sexual life (ED-NS), so as create an evaluating questionnaire, Self-estimation Index of erectile function-No sexual life (SIEF-NS) so as to investigate its clinical reliability. Methods The conception of ED-NS was identified and the SIEF-NS questionnaire was established. Patients who complained of ED-NS and normal controls were enrolled into the research and assessed the erectile function was assessed with SIEF-NS.The SIEF-NS includes 12 questions, such as sexual libido, general erectile function, nocturnal penile erection, erectile function during foreplay, erectile function during audio-video sexual stimulation,confidence, depression, etc and each question has 5 point scales. Results Sixty-one ED-NS patients and 57 controls were enrolled into the study and assessed erectile function with SIEF-NS. The mean score of each question and integral score of SIEF-NS in ED-NS patients were significantly different from normal controls (P<0. 05). When the integral score was 35 points according to the ROC curve of integral score, the sensitivity of SIEF-NS was 88.5% and specificity was 96. 5%. Conclusions ED-NS is a new conception to define patients who have erectile dysfunction without sexual life. SIEFNS is suggested to be a useful method for the evaluation of ED-NS patients.

ABSTRACT

Objective To assess the causes of sildenafil failure and the feasibility of successfully rechallenging non-responding patients. Methods A total of 126 consecutive erectile dysfunction (ED) patients from Jan 2009 to Dec 2009 who claimed poor response to sildenafil (sildenafil 100 mg on demand, at lease 4 epiodes) were enrolled into the study. All patients received sexual reeducation and were treated with sildenafil, taken on a daily dose of 50 mg for 4 weeks. The International Index of Erectile Function-5 (IIEF-5), Rigiscan, serum testosterone or penile Doppler were used to evaluate ED and the cause of on-demand sildenafil failure. End point efficacy of rechallenging was evaluated using the IIEF-5 and the sexual encounter profile (SEP) 'Were you able to insert your penis into your partner's vagina?' and 'Did your erection last long enough to achieve successful intercourse?'. Results The recruited patients comprised of 41 cases with psychological ED, 39 cases with hypogonadism ED, 28 cases with diabetes mellitus ED and 18 cases with vascular ED. Compared with pretreatment and on-demand sildenafil baseline, daily administration of sildenafil significantly enhanced all efficacy outcome variables. The IIEF-5 was significantly improved after daily sildenafil (12.3 ± 2.9 vs18. 8±4.4, P<0.01), 78 patients responded to daily sildenafil. The overall salvage rate was 61.9%(78/126). Conclusions Sexual reeducation and daily administration of sildenafil may be able to salvage many patients with ED who were sildenafil non-responders.

ABSTRACT

Objective To estimate any influence of strong pulsed magnetic fields on the expression of growth-related genes in human bladder cancer BIU-87 cells. Methods Human BIU-87 cells were cultured in vitro and randomly divided into a magnetic field group and a control group. Each group was further divided into 24 h, 48 h and 72 h sub-groups. The magnetic field group cells were exposed to an 8 T magnetic field pulsed at 15 Hz for 2 h every day. The control group cells also placed on the same environment, but not exposed to any strong, pulsed magnetic field. The expression of B cell lymphoma/leukemia gene-2 (Bcl-2) mRNA, Bax mRNA and caspase-3 mRNA was measured with RT-PCR, and flow cytometry was used to evaluate the expression of the Bcl-2, Bax and caspase-3 genes of the tumor cells in vitro. Results The expression of Bax mRNA and protein was significantly higher in the cells exposed to the magnetic field than in the control groups. The expression of Bcl-2 mRNA and protein was significantly less. The expression of caspase-3 mRNA and protein in the two groups showed no significant differences.Conclusions A strong, pulsed magnetic field can inhibit the growth of bladder tumor BIU-87 cells and promote their apoptosis. The mechanism is probably related with the magnetic field promoting Bax mRNA and protein expression and inhibiting Bcl-2 mRNA and protein expression.

ABSTRACT

Objective To assess the causes of erectile dysfunction (ED) after spinal or pelvic injuries.Methods From 2005 to 2009,a total of 67 patients who complained of ED after pelvic fracture or spinal injures were evaluated with the average patient age of 32 yrs (18 to 64) and average injury period of 38 mon (3 -144).Of the 67 patients,56 had pelvic fractures (84%) and 11 patients had spinal injures ( 16% ).All patients reported normal erectile function before the injuries.All patients answered IIEF-5 and underwent nocturnal penile tumescence (NPT) tests (Rigiscan assessment system,continuing three nights).If resuls of the test were abnormal,color penile duplex ultrasound ( CDU ) with intracavernous injection were performed.Normal nocturnal erectile function was defined as at least three tumescence periods lasting more the 10 minutes with rigidity at the penile tip of at least 70%.Patients with abnormal nocturnal erectile function were diagnosed with organic ED.Patients who achieved a fully erect penis after intracavernous injection,and who had a peak systolic velocity ( PSV ) of less than 25 em/s on CDU were diagnosed with arterial ED.Patients with a PSV of more than 25 cm/s and an end diastolic velocity (EDV) of more than 5 cm/s on CDU indicated venous leakage.Patients with organic ED who had normal vascular function on duplex ultrasound were diagnosed with neurogenic ED.Results All patients reported IIEF-5 less than 8,NPT tests showed that all patients had organic ED.Of the patients with pelvic fracture,neurogenic ED,arterial ED and venous leakage was diagnosed in 24 patients (43%),22 patients (39%) and 10 patients (18%),respectively.However,the patients with spinal injuries were all diagnosed with neurogenic ED.Conclusions Pelvic fracture and spinal injury can cause organic ED.Vascular ED is common in pelvic fracture,however,neurogenic ED was the most common type of ED in spinal injury.

ABSTRACT

Objective To compare the circumferential approach and suprapubic approach for correcting the concealed penis. Methods Thirty-four patients received circumferential approach (group A,18 cases)or suprapubic approach(group B,15 cases)randomly for the concealed penis.The length of penis without erection before and after surgery treatment,patients'satisfaction and surgical complications were evaluated.All the patients were followed at least 6 months after operation.Results The penile length before and after surgery in group A were(1.78±0.41)cm vs(3.97±0.47)cm,the length of the postoperative penis was significantly longer than that of the preoperative penis(P<0.01).83%of those in group A(15 cases)were satisfied with the cosmic results.The length of preoperative and postoperative penis in group B were(2.07±0.53)cm vs(4.05±0.81)cm respectively,the length of the postoperative penis was significantly longer(P<0.01).Satisfaction was 87%(13 cases).The postoperative penile length between the 2 groups was not different(P> 0.05).Prepuce edema was reported to Occur in 56%and 13%in group A and group B and spontaneously disappeared within 3 months.Fat synchysis occurred in 2 of group B. Conclusions The cosmic effect of the 2 surgical approaches is similar,but circumferential approach is simpler.There is no serious complication in those receiving circumferential approach.

ABSTRACT

Objective To evaluate the efficacy of microvasoepididymostomy for the treatment of obstructive azoospermia. Methods Ninety-eight patients were confirmed as obstructive azoospermia by infertility investigations.The mean (range) age was 31 (20-43) years.The mean duration of obstruetion was 4 years.All patients were seen by the surgeon for a complete history and physical examination.Semen analyses proved azoopermia,serum levels of sexual hormone were normal and testicular function for production of sperms were normal certified by biopsy.All patients with suspected epididymal or vasal obstruction were offered scrotal exploration.The decision for microvasoepididymostomy was made during surgery,based on the pateney of the vas towards the abdomen documented by saline and sperm fragments containing in the epididymal fluid on intraoperative light-microscopic examination.Microvasoepididymostomy was performed in 58 patients documented epididymal obstruction The initial semen analysis was then done after usually 3 months postoperatively.Patency was arbitrarily defined as>10 000 sperm/mL in ejaculate in at least one semen analysis after surgery.Postoperative patency rate and postoperative impregnation rate were followed. Results Fifty patients were followed up for 3-29 months,8 cases lost.Sperm was found by semen analysis in 36 patients.Sperm density was 4×104-2×108 sperms/ml and motility was 2%-70%.Semen analysis reveals azoospermia in 6 patients and the patients were counseled to undergo further testing to determine the ultimate outcome of the procedure.Natural conception occurred in 14 patients followed for more than 12 months.The overall pateney rate was 72%(36/50).Among patients with a follow-up of>6 months,the natural paternity rate was 28%(14/50).The median time to achieve a natural pregnancy was 6.6 (4.0-10.0)months. Conclusion A more favourable patency can be achieved using microsurgical intussusception vasoepididymostomy and a part of patients with obstructive azoospermia can be cured by this means.

ABSTRACT

Objective To study the changes of T-C of apolipoprotein CⅢ (ApoCⅢ)-455C gene and to de-tect the relationship between essential hypertension (EH) and the quantity of ApoCⅢ-455C gene. Methods 606 subjects were divided into hypertension group(n=306) and non-hypertension group(n=300). And the two groups 606 were divided into subgroup A (n=90) who had hypertension companied with hypertriglyceridem, subgroup B (n=216)who had hypertension only, subgroup C (n=51) who had hypertriglyceridem only and subgroup D (n= 249) who had none of the two diseases, by the concentration of triglycerides (TG) (TG≥1.69 mmol/L). The quan-tity of ApoCⅢ-455 C was detected by Real Time PCR. TC, TG, HDL-C, LDL-C, ApeA, ApoB 100, ApoCⅢ, ApoE, blood glucose,plasma insulin and waist circumference were measured in all cases. Results There was no difference in the quantity of ApoCⅢ-455C gene between group B and the group D [(45.16±5.97) vs (46.21±6.13)] (P>0.05). The concentration of ApoCⅢ, ApoB100 and the quantity of ApoCⅢ-455C gene in group A were higher than those in group B [0.14±0.03]g/L, (95.00±15.69)g/L, (28.13±4.11) vs (0.09±0.03)g/L,(81.22± 11.87)g/L,(45.16±5.97)(P<0.01)]. It was similar in ApoE [(0.05±0.01) g/L vs (0.04±0.01)g/L] and LDL-C [(2.54±0.61)mmol/L vs (2.15±0.41)mmol/L] (P<0.05). Conclusions The quantity of ApoC Ⅲ-455C gene is not related to EH.

ABSTRACT

Objective To review the clinical manifestation,pathological characteristics,treat ment and prognosis of malignant mesothelioma of the tuniea vaginalis testis.Methods A case of ma lignant mesothelioma of the tunica vaginalis testis was reported and relative literature was reviewed.A34 year old man was admitted to hospital with a history of aggravated hydroeele for 5 years.A hardtumor was palpated above the right testis,which was smooth and slight tenderness.The level of β-hCG and AFP was normal.Ultrasonography revealed a 2.05 cm× 1.97 cm,well demarcated,mixedecho tumor.Results An operation was performed to incise the tumor and tunica vaginalis undersubarachnoid space block anesthesia.The tumor was histologically diagnosed as low potential malig nant mesothelioma of the tunica vaginalis testis.Microscopic examination showed that the tumor wasmalignant with a biphasic pattern composed of epithelial and stromal components.The former compo nent was dominant.The epithelial cells proliferated obviously with atypia and mitosis.Immunohisto chemical staining showed cytokeratin and vimentin were strongly positive.The capillary,lymph ves sel,spermatic cord and testis were not infiltrated.Right radical orchiectomy and right scrotectomywas performed one month after the first operation.There was no evidence of relapse during 38 monthfollow up after operation.Conclusions The major symptom of patients with malignant mesotheliomais hydrocele,malignant mesothelioma is diagnosed by histopathology.Due to the invasive potential ofthis disease and the risk of tumor recurrence,radical orchiectomy and close follow up are strongly rec ommended.

ABSTRACT

Objective To investigate the expressions of CD40 and ICAM-1 in primary hepatocarcinoma and clinical significance and analyze their relationships with carcinogenesis and progression of liver cancer. Methods S-P immunohistochemistry was used to detect the expressions of CD40 and ICAM-1 in 40 cases of hepatocarcinoma and 25 cases of normal liver tissue (distance from cancer tissues≥2 cm).Results The positive expression rates of CD40 and ICAM-1 were 47.5% and 82.5% in liver cancer tissue,which was significantly higher than those in normal liver tissue (P0.05).The positive expression rate of ICAM-1 was associated with lymph node metastasis as well as histological grade(P
